Fort Howard where Vondas and the Greek throw their cell phones into the bay while being surveilled by Bunk and McNulty in S5. I've sailed past here hundreds of times, and the view and location are unmistakeable. They were standing on the WSW part of the point, not accessable to the public. The fixed red aid to navigation you see in one pan also confirms the location.
Viva House where Bubbles works the soup kitchen in Season 5. West side, a few blocks north of Wilkens Ave, corner of Mount and Boyd.

This is the Episcopal church where Bubble attends NA meetings. These red doors are prominent in the outdoor scenes, and are how I found the location.
Fictional scene of the basketball game in S1. You can hear someone on the radio give the location as "near Cloverdale", and this court is in the little park between Cloverdale, McCulloh, and Druid Hill. So it's on the East side which is correct for the show. I found out it was actually filmed on the West side at Collington Sqare (http://maps.google.com/maps?q=&sourceid=navclient-ff&ie=UTF-8&rlz=1B3GGGL_enUS290US290). Here's the East side map link: http://maps.google.com/maps?q=&sourceid=navclient-ff&ie=UTF-8&rlz=1B3GGGL_enUS290US290
Alleged (elsewhere) location of Kavanaughs, but the building doesn't match the one in the show. Kavanaughs is the bar where they hold the police wakes, including the one for Mcnulty in the last episode of S5.

1. Collington Square Park (Corner of Collington Avenue and East Hoffman Street) “This is the East Baltimore version of Federal Hill,” Peranio says. “One of my favorite views in the city.” Perched atop a hill, the large park offers sweeping vistas of the Johns Hopkins medical complex. The basketball court here was the site of the East-West game in the first season, between teams managed by east-side drug captain Proposition Joe Stewart and the west-side crew run by Avon Barksdale. The park’s grounds are also often used as locations for clandestine meetings between drug-dealer characters.

This is where Brandon (Omars boy) is picked up by Avon's muscle (S1 E5) and subsequently tortured to death. The actual location is directly across the street, as you can see when Wallace and Poot are walking on the street.
Mcnulty and an ME have breakfast here, can't remember which episode. The Medical Examiners building is across the street. Penn and Pratt.
